| Invitation to List in the Bonitas Suppliers and Service Providers Database |
| |
| Bonitas Marketing, the marketing arm of Bonitas Medical Fund, is developing a suppliers and service providers database, which will be used as a source of reference by all of the fund’s departments. We would like to give all suitably registered companies the opportunity to be included in this database, and invite them to submit their details to the fund for inclusion. |
| |
| Enterprises owned by historically-disadvantaged individuals, women, young people under the age of 35 (“youth”) and people with disabilities are specifically invited to apply. |

| Providers offering goods and services in the following categories will be listed: |

Advertising (in both the electronic and print media) |
4. |

| |

| In order to qualify for listing, providers will need to submit a completed registration form, company profile and supporting documentation by 5 February 2010. |

| The following criteria will be used to evaluate applicants for listing: |
- Company profile and technical abilities
- Ownership by historically-disadvantaged individuals, women, youth and people with disabilities
- SMME status
- The supply of a valid SARS tax clearance certificate
